If you are interested in competing in intercollegiate athletics for the U.S. Air Force Academy, please click on the sport you wish to compete in below and complete the form.

When you submit the form it is sent directly to the head coach of the respective sport. This form is NOT an official university application. Please be aware that this is not a secure form.

So you are aware, NCAA rules and regulations prohibit coaches and administrative personnel from sending letters or electronic mail to high school students until after September 1 of their junior year in high school. While we are not permitted to write or e-mail you if you are not eligible per the above restriction, you are permitted to telephone coaches and administrators without restrictions.

Those who are freshmen or sophomores in high school are also permitted to contact the admissions office for information regarding U.S. Air Force Academy and its academic programs. You may call our admissions office at 1-800-443-9266 or write to:

WARNING: If you are a student-athlete enrolled at another four-year institution or if you have been enrolled at another four-year institution in the last year (Division I, II or III), NCAA rules and regulations strictly prohibit us from having any communication with you, directly or indirectly, until that institution grants our institution permission to contact you. Authority: Title 10, United States Code 9331. The information you submit to the Air Force Academy Athletic Department is used to assist you with the appointment process. This information is submitted to the Air Force Academy Selection Committee and is used to determine your suitability for Athletic Department endorsement as a prospective intercollegiate athlete. Upon completion, your Athletic Department Candidate Endrosement File may contain the following: Athletic Questionnaire; copies of congressional correspondence; copies of recommendations you may submit; copy of high school transcript and special testing data; medical examination results; film review analysis.

Disclosure is voluntary. However, since the "Whole Person" concept of appraisal is used, failure to provide requested information will result in your disqualification for an Athletic Department endorsement.
